Research Overview

Tesamorelin (trans-3-hexenoic acid-GRF(1-44)-NH₂) is a stabilised analogue of endogenous growth hormone-releasing hormone (GHRH) with a modified N-terminus that confers resistance to dipeptidyl peptidase-IV (DPP-IV) degradation, substantially extending its half-life compared to native GHRH. Tesamorelin holds the distinction of being the only GHRH analogue to receive FDA approval, granted in 2010 for reducing excess abdominal fat in HIV-positive patients with lipodystrophy (EGRIFTA®, Theratechnologies Inc.). This regulatory status reflects an extensive clinical evidence base. The pivotal Phase 3 trials (Falutz et al., NEJM 2007) demonstrated significant reductions in visceral adipose tissue as measured by CT scan, alongside improvements in lipid profiles and patient-reported outcomes. Research interest in tesamorelin has expanded to examine its effects on cognitive function. A notable randomised controlled trial (Baker et al., 2021) investigated tesamorelin's effects on cognition in older adults with mild cognitive impairment, reporting improvements in executive function and verbal memory — hypothesised to be mediated through IGF-1 signalling in the central nervous system.
